All plans:

• Use the same BCBST network of providers (Blue Network P in Tennessee and Bluecard PPO in all other states)
• Cover in-network preventative care at 100%
• Include prescription drug coverage

Signature PPO

• This is the richest plan
• You’ll pay the most out of your paycheck but receive the highest level of benefits
• It has the lowest deductible of the three plans, and you’ll pay a copay for office visits as well as for most prescription drugs
• Prescription drugs are not subject to a deductible

Choice + HSA Plan

• The middle plan when it comes to per-paycheck costs and deductibles
• This is a High Deductible Health Plan which means:
  • You’ll pay the cost of services, including prescriptions, until you meet the deductible
  • To help pay for those costs, you can set aside money in a tax-advantaged Health Savings Account (HSA), or save these funds for future needs
  • Ingram contributes to your HSA annually. $500 for individual coverage and $1,000 for family coverage

Value + HSA Plan

• A good option if you’re looking to pay a lower premium
• This is a High Deductible Health Plan, so you’ll pay the cost of services, including prescriptions, until you meet your deductible
• Lowest per-paycheck cost, but the highest deductible and out-of-pocket maximum
• You can apply the money you save per-paycheck to your HSA account, so you’ll have funds to cover medical costs when they arise
• Ingram does not contribute to your HSA for this plan

Telehealth

Don’t stress about having to go to the doctor! Telehealth visits are covered under your plan. This includes visits with your primary care physician, specialists, behavioral health providers, and physical, speech, and occupational therapists. The visit will cost the same as a face-to-face visit with your provider, so any copay or cost-share you would normally have will still apply. Call your regular provider to see if they offer telehealth visits.

Mental Health and Substance Abuse Coverage

If enrolled in an Ingram medical plan, you and your eligible dependents have access to mental health and substance abuse coverage through BlueCross BlueShield of Tennessee.
