- After you meet the deductible, the plan pays a percentage for each eligible service—you pay the remainder
- See the provider of your choice. However, you will lower your out-of-pocket costs if you use a dentist who is a member of one of the plan’s two networks, Delta Dental Premier and Delta Dental PPO

You must log in each time you access the secure portion of the app. No personal health information is ever stored on your device. For more details on security, our Privacy Policy can be viewed by clicking the lock icon on the main menu.
Cigna Dental Plan
- Provides you with coverage through a dental HMO (DHMO*), with no deductibles to pay
- Plan pays for all diagnostic and most preventive services received from a Cigna DHMO provider
- For basic and major services and orthodontic treatment, you pay a set copayment for each procedure, and the plan pays the rest (check the K1-09 fee schedule for current costs)
- All covered services must be obtained from an approved Cigna DHMO provider or by referral from that provider. Before receiving treatment check that your provider is in the DHMO network
